Glucagon

  1. Therapeutic Effects
    1. Hormone - hyperglycemic agent

  2. Indications
    1. Documented symptomatic hypoglycemia (BGL < 60mg/dl) when an IV cannot be established.
    2. Beta Blocker overdose with serious signs and symptoms (ALS)
    3. Anaphylaxis refractory to epinephrine, or in patients who have history of serious coronary artery disease.(ALS)

  3. Contraindications/Precautions
    1. Patients who will be unable to receive supplemental glucose orally, IV, or rectally after administration of glucagon.
    2. Hypersensitivity to pork and/or beef
    3. Use with caution in patients with pheochromocytoma

  4. Side Effects
    1. Tachyarrhythmias

  5. Administration and Dosage
    1. Hypoglycemia:
      1. Adult: 0.5-1.0 mg IM
      2. Peds: 0.25-0.5 mg IM
    2. Beta Blocker Overdose:
      1. Adult: 3-10mg IVP over 1 minute. May be followed by an infusion of 2-5 mg/hr.
      2. Peds: 0.1 mg/kg IVP over 1 minute, repeat in 5 minutes if needed.
    3. Anaphylaxis:
      1. Adult: 1-2 mg slow IVP, may repeated every 5-10 minutes.
      2. Peds: Rarely Indicated
